PetSmart Charities® Grant-Reporting Documents

The following are the various reporting documents that we require as part of receiving a grant. Depending on the type of grant, you may need to complete and submit one or more of the following:

Interim Impact Report – This report is to provide an update to PetSmart Charities® about midway through your project and typically used for those grants that are funded for more than one year. An expense worksheet should also be submitted with the interim report to provide a detailed listing of how the grant funds were used to date.

Final Impact Report – This report is to provide report data to show how the project achieved the expected results. An expense worksheet should also be submitted with the final report to provide a detailed listing of how the grant funds were used.

Expense Worksheet – This template is a user-friendly tool for grantees to itemize the project expenses and detail how the grant funds were used.

Authorized Representative Form – This form needs to be signed by an authorized representative from the grantee organization.

 New PetSmart Charities now provides the opportunity for all grantee organizations to submit the above reference reporting documents online! As the due date for your report approaches, you will either receive an e-mail or phone call from a PetSmart Charities Grants team member. This is dependent primarily on how the application was originally submitted or when the grant was awarded.

 

What’s the process?

  • If you receive an e-mail: You will be directed to your CyberGrants account login page. Once you log in, you will see the outstanding reports for a grant on file that are available for you to complete. The link to these templates provides you with the most current reporting documents that also appear on our website. Once you complete the report document/s, simply save and upload the document by clicking on the corresponding upload field. The PetSmart Charities program manager will review the content of your report and determine if any additional information is required. If additional information is required, the online report may be sent back to you electronically for updates. If no additional information is needed, the report required will be considered satisfied.
  • If you receive a phone call from a PetSmart Charities program manager: You will be asked to respond to questions that are contained within the reporting documents. The program manager may direct you to this website page for additional information and explanation for what is required. The program manager will ask you to complete the appropriate report document and provide further guidance on how to submit this information directly to them via email. The program manager will then upload the documents you send. The PetSmart Charities program manager will review the content of your report and determine if any additional information is required. If additional information is required, the online report may be sent back to you electronically for updates.  If no additional information is needed, the report required will be considered satisfied.
